Boot Camp Downloads for Recruits of Sustainable Fitness
The PDF files below have been provided so that you can download required forms and other important information about camp. A hard copy "ParQ & Waiver” form must be downloaded, printed, successfully completed and brought with you to the first day of camp so that you may participate in camp. In order for you to have successfully completed the online registration process, you would have answered “NO” to all ParQ questions online. If when filling out the hard copy of the ParQ form downloaded, you answer “YES” to any of the questions because of a change in your health, we ask that you download the PARmed-X form, bring it to your physician to complete, then bring us the completed form to your first day at camp so that you may participate in boot camp, as it is a requirement.

There are other helpful downloads that we ask you to review, such as what to bring to camp, refund policy, make-up day policy as well as information about our personalized water bottles. Maps to boot camp locations also contain specific information regarding entry and meeting places. Alternatively some forms have been made available to address specific situations, such as in the case where a participant is expecting a child, or medical concerns that must be addressed through a physician; a result of answering "Yes" to one or more of the medical questions on the "Par Q" form.
